Student Registration Forms
Please bring the registration packet, birth certificate, social security card,
and immunization records to the school or district office during office hours.
School Supply Lists may be found on your school's website.
The Power of Attorney form must be completed and notorized if you are a non-custodial parent and wanting to register a student into any of the Weld RE-5J schools.
