DOES YOUR CHILD HAVE A SERIOUS ALLERGY?
The district has adopted a plan to manage serious allergies at school. If your child has a serious allery, please see the following form. Please print the form to take to your physician to complete or you may speak to your school nurse to have it completed with your input. New Student Registration
Please bring the registration packet, birth certificate, social security card,
and immunization records to the school or district office during office hours.
