Lunch Application Carryover
2011/2012 school year lunch applications will expire on September 28, 2012.
Your child will change to paid on September 29, 2012, unless you fill out
a new application and are approved for free/reduced price meals for the
2012/2013 school year. You will receive a letter by mail from
Weld RE-5J School District stating your free/reduced approval or denial.
The Weld County School District RE-5J encourages ALL families
to fill out the lunch application. All information is confidential.

The application is available via the link above or at each school from the school secretary. You may turn in the completed application to any of the schools or you may mail it to:

Roosevelt High $2.20
Milliken Middle $2.05
KQA grades 6 – 8 $2.05
Milliken Elementary, Letford, Pioneer Ridge & KQA K – 5 $1.90
Extra Milk $ 0.40
